Seeing a physical therapist—in person or via telehealth—has never been more accessible. Most insurance policies include options for physical therapy, and thanks to direct access, you can often see a PT without a physician’s referral to address aches, pains, or mobility issues. telehealth.hhs.gov+1

Key dates (Marketplace/Open Enrollment)
- November 1, 2025: Open Enrollment begins. You can enroll in, renew, or change Marketplace plans. Coverage can start January 1, 2026. CMS
- December 15, 2025: Last day to enroll or change plans for Jan 1 coverage. HealthCare.gov
- January 15, 2026: Open Enrollment ends in HealthCare.gov states. After this date, you generally need a Special Enrollment Period to change plans. HealthCare.gov
- February 1, 2026: Coverage starts for those who enrolled Dec 16–Jan 15 and paid their first premium. HealthCare.gov
Will my plan cover physical therapy?
Under the Affordable Care Act, all new individual and small-group plans must cover essential health benefits, which include rehabilitative and habilitative services and devices (the category that encompasses physical therapy). Exactly how much is covered (visit limits, copays, deductibles) varies by state and by plan, so compare details before you enroll. CMS+2HealthCare.gov+2
Telehealth: what to know
Many plans include telehealth options for PT. Medicare’s expanded telehealth flexibility currently runs through September 30, 2025; policies after that date may change unless Congress extends them, and commercial plan rules can differ. Direct access to PT (no referral in most cases)
In North Carolina, where we practice, direct access is recognized by the state board. That means you can usually start with us first, and we’ll coordinate with your physician when needed. APTA+1
