You will need to register prior to being seen for your initial visit. We ask that you arrive 10 to 15 minutes prior to your initial appointment time to ensure ample time to fill out the registration forms.
Alternatively, the links below provide access to the patient registration packet forms. You may download, print, and fill out the appropriate forms prior to your initial visit to shorten the registration process. If you choose to fill out forms prior to your visit, please bring the completed forms with you. You may fax (888) 268.1042 or email angie@cedarhillpt.com the completed forms to us as well. While this will expedite the registration process, we still ask that you arrive 10 minutes prior to your initial appointment time.
For each visit after your initial appointment, you may arrive at your scheduled appointment time.

Most insurance companies pay for physical therapy. Some may require a doctor’s referral, and some allow you direct access to a therapist without a referral. We will help you sort through the insurance maze so that you will receive the greatest benefit from any coverage you may have. We will file your primary and secondary insurance for you. In order to do this, we require that you bring your health insurance card(s) to your initial visit for us to correctly file your insurance claims.
