If you are a physician or other healthcare provider who would like to refer a patient/client to Cedar Hill Physical Therapy, please feel free to download our Cedar Hill Physical Therapy Referral form. This form can be emailed, mailed or faxed to our office, or it can be given to the patient/client for them to bring to their initial visit.
